You may chose to download required forms to review before filling them out in the office 

or you can print and complete them prior to arrival.

First time clients download the first four required forms. 

If receiving hot stone, cupping or providing consent for a minor to receive massage please download respective forms.

If you are running late to your appointment please call and tell us as all no call/no show clients appointments will be canceled 

15 minutes after scheduled appointment time. 

Cancelation Policy:  Please cancel 24 hours prior to designated time.
